Membership Form
Please make sure you have digital copies of the documents required ready for uploading. These will include, depending on your circumstances, COVID vaccination, Aged Pension card, dog’s current vaccination certificate. This form must be completed and your payment processed to secure a place in our next Induction Session.
Please Note – you cannot save a partially filled in form to complete later, and leaving this form open to come back to later may result in your computer experiencing connection issues.
Please only submit the form once. If you experience any problems when submitting, please check your bank account before resubmitting the form. Multiple submissions may result in multiple debits to your account.
Once you submit the form, you will receive a Payment Confirmation Receipt email within a few minutes. If you don’t get an email, reach out to us at admin@woodvilledogtraining.com.au including the date and time you submitted the form.
We will investigate the issue and contact you with the outcome.
